Ok, so please correct me if I'm wrong DC, but this is I assume how you both dying to msto happened.

1) DeathCow is in the room before you, already casting msto.
2) You enter the room he is currently casting in and both engage to backstab a monster
3) Because backstab happens before spells and normal combat, both of your combat rounds go off before DeathCow's msto on the first round you saw.
4) DeathCow's manastorm hits you on the first round for 190 damage.
5) On the second round of combat, because DeathCow was in the room before you, his spell goes off before your combat rounds do.
6) Hoji's 'drops to the ground' and 'is dead' messages appear just before the 152 damage from DeathCow's spell is visible. Hoji is being hit by the same manastorm that killed you at this point. The message order is off, but the result is the same.

DeathCow did not do two manastorms in a single round. Other than the message order on that second round, everything you saw can happen on regular mud. The message order may even be the same in regular mud, it has been too long since I played regular to give you a reliable answer to that.
